Officials at the Department of Children’s Services say they have been working over the past three months to create a new process for releasing information to the public about children who died under their watch.

Report of the independent child death review group

The report does not make any findings in relation to any individual  This report is based solely on a review of case files provided by the HSE and on information obtained from Coroner’s office in a small number of cases where the ICDRG did not receive a Death Certificate from the HSE in repsect of children & young people whose cases are reviewed in this report. Any implicit or explicit value judgements are being made solely on the basis of the available records.
